New M11q Goes Into Effect in NYC on April 1, 2010 - Not an April Fool's Joke!

This is not an April Fool's joke!  Beginning today, April 1, 2010, M11q forms must be submitted to CASA offices in NYC using a new form, which is a revised version of the 20-year old form.    In this article, Applying for Medicaid Personal Care Services in New York City, you can download:

• An electronically fill-in-able copy of the new M11q form - M-11q Medical Request for Home Care  (HRA's official copy is posted at http://nyc.gov/html/hra/downloads/pdf/M11q.pdf  along with other information about home care  

• A revised copy of Q-Tips, Selfhelp Community Services' guide to the new M11q with tips about completing the new form

• A suggested template for physicians to add a comment page to the new form -- Supplement to the Medical Request for Home Care (M11q).  NOTE: We no longer recommend submitting this Supplement with the M-11q, because HRA has been rejecting them.  It is better to incorporate additional comments about ADLs into the M-11q itself, and/or add an additional plain sheet of paper to continue the comments from section G of the form.  See our article on applying for personal care in NYC for more info.
